Digital Data Submission Information (DDSI) Form

An Online Digital Data Submission Information Form (formerly "T2 Form") for ATC-supported protocols is available from the link below. This form is to accompany all digital data submissions to the ITC, whether for protocol case data or for credentialing (dry-run, benchmark, or phantom experiment) data submissions.

Note Regarding Rapid Review Submissions (applies to the following protocols only)
  • NSABP B-39 / RTOG 0413 PBI Cases:  (first case for each treatment method)
  • RTOG 0236
  • RTOG 0418
Please be aware that processing of a Rapid Review submission cannot begin until all required data have been correctly received by the ITC.  Once all data have been correctly received, the approval process will take up to three business days to complete.

Please do not submit printed hardcopy of the online form. If you are unable to submit the DDSI online, please contact the ITC for an alternative (PDF) form. The completed form is to be mailed or faxed to the ITC at the following address:
